Any SDC chapter that hosts an event or meet and needs an Insurance Certificate should submit the SDC Insurance Form. To download the insurance form PDF, CLICK HERE.
Fill out and mail your form to:
Please allow 30 days for turn-around time.
The coverage for this insurance is $3,000,000 and is provided to chapters at no cost.
Chapter Officer Liability Insurance is separate insurance that covers the elected and appointed officers in chartered SDC Chapters. There is no cost for this insurance. Obtaining the insurance requires that the chapter send in a current list of officers. For complete information and the form, CLICK HERE.
